I don’t see the utility with distributing funds to an “eICU team” to basically double up on patient care. I’d be interested in seeing the research comparing just adding an additional float nurse in-person, versus an e-ICU team. There is no way a tele-doc would order our patients to be intubated based on a screen.

I think Ben Daxon has made a good presentation and done his research, however, I am a retired respiratory therapist with 30 years of bedside experience with ventilators. I have worked more than 1000 hours with patients on APRV. I recommend against APRV in all patients. Can APRV work in some situations? Yes. Is it a mode you can rely in day in and day out? No. Which means APRV is a crap shoot. A guess. Not the best way to manage vent patients. When I manage vents, I know what is going to happen and how I'm going to wean. Managing patients should not be a guess. You should know.

Good information. My husband just had a 28 day icu stay...many of these preventative measures were used. The first few days he was very paranoid and didn't trust his caregivers. I was only allowed 1 hour with him every day. I finally left him a note to reassure him that he was safe and that the people who came in were there to help him. They did well with nutrition and early mobilization. Hand off was not super...but I am a nurse and could fill in the blanks for rehab. His cognitive is back to baseline...physical not yet.
